I had an MTB mostly ridden on street, had a slightly bent rear axle (jumping curbs) which lead to bearing cones getting pressure and then exploding and jamming the hub, which tried to elongate the quick release. Once the quick release snapped the wheel came up the frame and got jammed sideways.

There was an indication something was wrong that day, I though when I got home I'd fix it. Instead I walked the wreck back.
