Unknown number calls my phone, I answer, woman on line says she represents "company xxx," says I failed to fill in one field on a form, can she ask me for the information so that the form will be complete? I ask, what's the information? she says, "are you a US citizen?"

"how did you get my phone number?" Then consider changing it, and going forward only using prepaid service and never giving your name.
This could be a social engineering attack from a scam artist or a non-governmental fascist making lists. Under no circumstances should such dangerous information be shared with a private company.
That includes hospitals here in FL: they are required by law to ask, but are supposed to treat you any way if you refuse to answer. Enough people must always refuse to answer that the cops cannot be summoned when migrants won't answer due to the volume of calls that would otherwise spam them.
